At Toronto Community Care Access Centre (CCAC), we continually focus on new ways to deliver excellence and help clients and their families get the health care services and support they need. As we implement several innovative new programs, we are looking for Registered Nurses and Care Coordinators to play a key role in meeting the needs of Torontonians. Whether you're an RN wishing to explore case management while using your hands-on skills in a different kind of health care environment, or you're a nurse or other health professional looking to bring your expertise and interdisciplinary focus to a case management function, this is an exceptional opportunity to join our team. Registration or certification in good standing with the appropriate professional college or association is essential for all roles, as is the ability to work with culturally diverse groups.
School Mental Health & Addictions Nursing Team - Parents, teachers and schools seeking to help students deal with their mental health and addictions issues will be able to turn to our School Mental Health & Addictions Nurses for health-related advice and strategies to recognize and address students' needs. Our nurses will also help students and parents access resources such as family health care, community mental health and/or addictions agencies. Applications welcome from RNs with related clinical experience. English/French bilingualism is a definite asset.
Rapid Response Nursing Team - The first 24 hours following a patient's discharge can often be stressful and filled with worry. Being able to call on our Rapid Response Nurses for additional health care support will make it easier for people with complex health needs to transition from hospital to home. Ensuring communication and linkage with primary care, this initiative will go a long way towards freeing up hospital beds in Ontario. Applications welcome from RNs with acute care or related experience, and especially those skilled in working with children with complex needs.
Alternative Level of Care (ALC) Team - - Finding the right place of care for clients is the primary mission of ALC Care Coordinators. Through cross-sector reviews, they determine the appropriate ALC destination for patients, examine any barriers, and develop sustainable transition strategies in co-operation with acute care and rehab hospitals, LTC homes, community support services, attendant care, supportive housing and other partners. Applications welcome from experienced RNs, MSWs, OTs, PTs, RDs and SLPs.
