At the Central West Community Care Access Centre (CCAC), we work in a collegial, client-focused environment and continually look for innovative ways to help clients and their families get the health care services and support they need. Two new initiatives to the CCACs - the Mental Health & Addiction Nurses in District School Boards Program and the Rapid Response Program - call for RNs with clinicial nursing expertise to provide leadership and direct care in meeting community needs in Brampton, Etobicoke and Orangeville. Client Services Manager (Nurse Leader), Mental Health & Addictions - In conjunction with the Mississauga Halton CCAC, we are seeking an achievement-oriented manager to lead the planning, development and implementation of the Mental Health & Addiction Nurses in District School Boards Program - from hiring and managing mental health nurses to advising school boards. This school-based program is important Ontario's plan to improve services and support for children and youth with mental health and addictions needs. Supporting both CCACs in a matrix reporting relationship, you will use your knowledge of the mental health and addictions service system for children and youth, direct clinical experience in this field, and proven skill in leading the development and implementation of new programs, monitoring KPIs, managing staff, setting priorities adn working with multiple health disciplines.
Registered Nurses, Mental Health & Addictions - Drawing on your strong clinical nursing experience and knowledge of the mental health and addictions service system for children and youth, you're ready to be part of the inaugural team of this exciting program. Assigned to a district school board, you will advise educators, develop individual care plans with other professionals, assist boards in building capacity and developing strategies to recognize and address students' issues and needs in this critical area, and help students and parents access services such as family health and/or addictions agencies.
Registered Nurses, Rapid Response Program - Play a key role in ensuring effective transitions from acute to home care for target populations, such as medically complex children and frail adults, and seniors with complex needs and/or high-risk characteristics such as congestive heart failure. With your 5+ years of relevant clinical experience, knowledge of community case management and direct care, and advanced diagnostic reasoning skills, you will make the first in-home nursing visit to high-needs seniors and children within 24 hours of their hospital discharge.
Registration as an RN in good standing with the CNO, the ability to work with culturally diverse groups, a valid driver's license and access to a reliable vehicle are required for all positions. English/French bilingualism is an asset. Full position details are available online at ccacjobs.ca.
