The South West Community Care Access Centre (South West CCAC) is dedicated to supporting and enhancing the quality of life, independence, health and well-being of individuals in the communities we serve by offering a single point of access for community services and demonstrating leadership and excellence in community care.
Located in Southwestern Ontario, the South West CCAC partners with clients and care providers, including hospitals, community support agencies, LTC homes and other to ensure appropriate care is available to clients.


Quality & Process Improvement Coach

Join the South West CCAC’s Quality & Process Improvement Team in supporting their system-level quality improvements and fostering a culture of continuous quality improvement. As the Improvement Coach, QPI, you will be responsible for enabling, supporting and sustaining improvement efforts within the QPI strategic and work plan.
Specifically, you will be responsible for: facilitating sessions to establish project/organizational goals and action plans; providing tools, templates, and education pertaining to continuous improvement; transferring of learning and performance monitoring; and establishing and maintaining strong relationships with various stakeholders.

Qualifications: The successful candidate will have: post graduate degree in healthcare, business administration and/or equivalent education and experience; experience in a lead facilitator and project management role; in-depth knowledge of quality management frameworks and methodologies (i.e. Lean, Six-Sigma, Institute for Healthcare Improvement, etc.); excellent knowledge of Ontario’s health system including primary, acute, community and long-term care settings; good understanding of the role of provincial quality improvement organizations and initiatives (i.e. Ontario Health Quality Council, Centre for Healthcare Quality improvement, etc.); superior communication and group facilitation skills; and strong collaboration and change management skills.
