At Toronto Central Community Care Access Centre (CCAC), we continue to focus on new ways to deliver excellence and are looking for several Client Services Managers to lead teams in a variety of settings - the community, a local hospital or the CCAC office - and help clients and their families get the health care services and support they need. Your passion for health care, your vision for excellence in client service delivery, and your strong operational and people management skills will drive your success. These are exceptional opportunities to lead and manage change by contributing new ideas or helping to develop new program areas. We want to hear from passionate, dynamic invididuals to lead the following teams:
SCHOOL MENTAL HEALTH NURSING TEAM - An RN with a strong clinical background and knowledge of the school mental health and addictions service system for children and youth, you're ready to get in on the ground floor of a brand-new and innovative program that will see you building, managing, and providing leadership and knowledge to a clinical nursing team.
TELEHOMECARE TEAM - Drawing on your clinical expertise as an RN as well as your knowledge of disease management, you will lead a team of nurses in this innovative new program. The focus is to provide health coaching and remote monitoring for clients and to encourage self-management of their complex chronic diseases.
HOSPITAL TEAM - If you excel at relationship-building within a health care environment, you will enjoy this opportunity to lead a team of hospital-based Care Coordinators whose role is to help transition clients from the hospital quickly and efficiently, so they can enjoy life again in the right place of care.
COMMUNITY TEAM - Your expertise with population-based models will enable you to really make a difference to specific client groups in the community. You will lead a team of Care Coordinators who conduct face-to-face assessments with clients in the community and connect them to the right community-based supports and care, ensuring they can continue to live safely within the community.
PROJECTS & OPERATIONS TEAM - These roles require team leaders who have a drive for excellence, the ability to identify areas for improvement and the project- and people-management skills to drive that change. We will look to your expertise and your team's support to manage various operations-related projects.
A facilitator and problem-solver with 3 to 5 years of experience managing in a multidisciplinary, culturally diverse health care environment, you will oversee the ongoing management, planning and evaluation of case management and health care coordination within your assigned area. These positions call for recognized relationship builders who can work with our diverse service partners, and motivate and inspire a dedicated CCAC client service team to ensure a commitment to continuous improvement and quality, client-focused service delivery. Committed to providing the highest level of customer service and client care, you will be accountable for the quality and fiscal management of delivered programs and services, manage a staff complement that may include remotely based team members, and identify opportunities to develop new programs and services to meet evolving health care needs. A graduate degree (or equivalent) in Social Sciences, Nursing, Health Administration or a related field is required. English/French bilingualism would be an asset for some positions.
